Team Description:
Our homecare teams are Interdisciplinary teams providing hospice services and support to patient and caregivers in their home environment.
Each team is divided up to serve a specific geographical area.
Each member of the Interdisciplinary team (Nurse, Social Worker, Chaplain, Nursing Assistant) provides discipline specific services within their scope of practice to their assigned patients, while working as a collaborative team to support our mission.
The Team Manager (TM) is responsible for managing the day-by-day activities of an interdisciplinary team including supervision, resource management, scheduling and assignments.
The TM ensures that all patient care is provided in accordance with established organizational procedures and government regulations.
The TM aids in recruitment process, develops, supervises, and trains clinical staff.
The TM also actively participates in on going process improvement.

Or the equivalent combination of education, technical certifications, training, and work experience.
A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
Minimum 2 years related experience required with increasing management responsibilities.
Related experience includes home health/hospice, oncology, and medical surgical.
Proficiency in Microsoft Office is required.
Recommended Skills Business Process Improvement Certified Nurse Practitioner Clinical Works Diploma In Nursing Health Care Hospice Estimated Salary: $20 to $28 per hour based on qualifications.
